DR SAEED MONTERREY, PT, DPT, OCS
Doctor of Physical Therapy Board Certified Specialist in Orthopaedic Physical Therapy Member of the American Physical Therapy Association
Dr. Saeed completed his undergraduate studies at the University of Wisconsin-Milwaukee, earning a Bachelor of Science in Kinesiology. He then moved to the Valley to pursue his Doctorate of Physical Therapy (DPT) at A.T. Still University. There, he met his lovely wife, Dr. Dayna Monterrey, and together, they co-founded Revive Physical Therapy & Wellness with a shared vision of elevating the community's healthcare by providing superior, evidence-based, and personalized non-invasive orthopedic treatment.
As a Board-Certified Specialist in Orthopedic Physical Therapy (OCS), Dr. Saeed specializes in spinal rehabilitation, sports injuries, and post-operative recovery. His treatment approach is rooted in hands-on manual therapy, creating the optimal environment for tissue repair, remodeling, and neuromuscular re-education. He also has extensive training in spinal manipulation/mobilization and the McKenzie approach, particularly for patients with spinal and extremity mechanical pain. Dr. Saeed firmly believes that movement is essential to health and that an active lifestyle is the key to long-term well-being.
Beyond his work, Dr. Saeed is a devoted husband and a proud father of three. He cherishes spending time with his children, whether running around and playing outside, watching their sports activities, or camping up north with the family-a favorite tradition that allows them to unwind and connect with nature.
Passionate about his profession, Dr. Saeed is dedicated to helping his patients regain function and find lasting pain relief, empowering them to live healthier, more active lives.

DR. ANDY MEZA, PT, DPT, AIB-VRC

Dr. Andy is an Arizona native from the border town of Yuma, Arizona. He spent his early years in Yuma before moving north to attend Arizona State University, where he earned a bachelor's degree in Kinesiology. As an active ASU Sun Devil, Dr. Andy took part in various volunteer opportunities in the physical therapy field and quickly fell in love with the profession after witnessing firsthand the profound impact skilled physical therapy can have on quality of life. After graduating, he moved to Mesa to pursue his Doctorate in Physical Therapy at A.T. Still University.
Dr. Andy has a strong passion for treating balance and orthopedic conditions. He enjoys connecting with his patients and educating them about their condition, empowering them to take an active role in their recovery. He firmly believes in the power of a personalized treatment plan and is committed to ensuring that each patient's individual goals are met.
When he's not in the clinic, Dr. Andy enjoys leading an active lifestyle. He has developed a deep passion for Brazilian Jiu-Jitsu, continuously training and refining his skills. He also enjoys strength training in his free time to stay fit and strong. On weekends, he loves visiting Yuma to spend time with friends and family or exploring new restaurants and food spots around town.

Cole Bembenek, PT, DPT
Doctor of Physical Therapy
Cole is a dedicated and passionate physical therapist originally from Rosholt, Wisconsin. He earned his undergraduate degree from the University of Wisconsin-La Crosse, where he developed a strong foundation in exercise science and rehabilitation. Following his undergrad, Cole pursued his Doctorate of Physical Therapy at Clarke University in Iowa, where he honed his clinical skills and deepened his understanding of patient care.
With a genuine commitment to making a difference in the lives of his patients, Cole believes in the power of personalized treatment plans that address each individual's unique needs. He focuses on developing real, sustainable change through a combination of evidence-based practices and compassionate care. Cole's approach emphasizes collaboration with patients, empowering them to take an active role in their recovery journey.
In his free time, Cole enjoys exploring the vast hiking trails the valley has to offer, seeking out local coffee shops, and exploring new restaurants. He also enjoys playing pickleball and spending quality time with friends and family in his free time.

Jose Garcia, PTA
Physical Therapy Assistant
While completing his Bachelors degree at ASU in Kinesiology, Jose pursued a career as a personal trainer, working closely with clients to help them achieve their personal health and fitness goals. During this time, he discovered the profound benefits of functional body training and the healing power of movement.
Motivated by his desire to make a positive impact on people's lives, Jose decided to pursue a career as a Physical Therapy Assistant. He completed his program through Gateway Community College first in his class and became a licensed clinical provider.
In his free time, he enjoys traveling to new places, spending time in the kitchen, immersing himself in different cultures, and trying new foods. He also loves spending time outdoors, particularly with his two dogs, Koda and Talulah, who are always up for an adventure.

Victoria Valenzuela, PTA
Physical Therapy Assistant
Victoria is a dedicated physical therapist assistant who graduated from Gateway Community College. Before transitioning to physical therapy, she spent four years working in Arizona's film industry after earning her bachelor's degree in film from Arizona State University. This unique background fuels her passion for discussing movies and TV shows, so don't hesitate to share some recommendations!
Victoria's interest in physical therapy was sparked in high school when she learned that a weak core made carrying a heavy drum in marching band incredibly painful. This experience deepened her understanding of the body's mechanics and the importance of strength and stability.
Motivated by a desire to help others regain their mobility and improve their quality of life, Victoria is committed to her role as a physical therapist assistant. She values the connections she builds with her patients and is dedicated to supporting their recovery journeys.
Outside of the PT clinic, Victoria enjoys tackling tough hikes, discovering new music at live events, and exploring exciting new cities. She believes that staying active and engaged with the world around her enriches her life and enhances her practice.

Dr Monterrey aims to enhance your overall quality of health, and works with you to achieve this by utilizing personalized treatment plans tailored to each unique need of the patient. She also emphasizes the importance of nurturing all aspects of health - physical, emotional and hormonal - to create harmony and long term well-being.
Beyond professional interests, Dr. Monterrey is a mother of 3, dog mother mother to 2 and embraces an active, fun lifestyle with her husband and children. Whether it's camping, biking, hiking or exploring the outdoors, she believes in fostering a healthy, vibrant life both at home and in practice.

Taylor is a proud Arizona native who recently graduated with her Master’s degree in Healthcare Administration from Park University. With a passion for improving healthcare systems and patient care, Taylor is committed to making a difference in the healthcare industry by improving patient care and organizational efficiency.
Outside of her career, Taylor enjoys a variety of hobbies that keep her life balanced and joyful. She shares her home with three beloved dogs—two playful Dachshunds, Lola and Booker, and a spirited Husky named G. Whether she’s hitting the golf course, experimenting with new baking recipes, or spending quality time with friends and family, Taylor values the moments that bring happiness and connection to her life.
With a warm and compassionate nature, Taylor is always seeking ways to grow both professionally and personally, making a positive impact on those around her.

Candace Mylar
Patient Care Coordinator
Candace is a driven and accomplished professional from San Diego, California. She graduated with Summa Cum Laude honors from Grand Canyon University, where she laid a strong academic foundation. With a passion for helping others, Candace became an EMT, gaining invaluable hands-on experience in the medical field.
Candace has built a successful career as a Patient Care Coordinator these past two years, where she excels in administration and patient-focused services. Her organizational skills and commitment to exceptional care have made her a trusted team member in healthcare settings.
Currently, Candace is pursuing her Certified Personal Trainer (CPT) certification to expand her expertise and help individuals achieve their fitness goals. In addition to her professional accomplishments, Candace is a dedicated and competitive athlete with a love for softball and basketball.
Outside of work, Candace enjoys spending time with her beloved dog, Samson, a playful and loyal Samoyed. She is always eager to learn and grow, driven by her commitment to improving both her personal and professional life.
